Clear cover is available at Lowe's or Home Depot, and is made by American Tack and Hardware.  This 2 part cover has a back that screws to the wall box, then you sandwich your embroidery in the middle, and snap the clear plastic cover over the top.  With this style of cover, you don't have to put screws through your embroidery.  Instead, your fabric covers up the screws.
